Primary Care Utah is an insurance program sponsored by the Utah Department of Health which provides primary care medical services and prescription medication coverage. Applications will be accepted for adults without minor children from February 1-27, 2018.

This is an excellent (and rare) opportunity to obtain coverage for medical care. If you meet the income limitations there are no monthly premiums and visits to a primary care doctor cost only $4 each, each prescription medication costs only $4 and dental care, vision examinations, birth control and many other services are provided at no cost.
How to apply
You can apply online at https://jobs.utah.gov/mycase/, download and print an application at that site or apply in person at your local Workforce Services office. The only requirement for enrollment in the program is that you meet certain income restrictions. For example, for a single individual to be eligible they cannot have monthly income over $955.00 per month or $11,460 per year. A family of 2 people cannot have monthly income over $1289 per month or $15,432 per year, and a family of four is $1948 per month or $23,376 per year. These are the 2018 amounts, the income limits change yearly. If you fall within these income limits and are uninsured, we strongly encourage you to apply for this program.
